Online Recital Live-Stream Consent form
This June online recitals will be live-streamed on Sarv Music Academy's facebook and Instagram accounts available to friends and followers of Sarv’s account on June 12, June 19, and June 20, 2021 depending on the instrument students play.
Please indicate whether or not you give consent for use of your/your child’s video for the livestream. This live-stream will only be up during the concert and will be deleted from Sarv’s videos after the end of the concert. Those who do not wish to appear on our facebook/instagram live-stream will still be participating in the online concert which only be available to those who participate in the zoom concert. Neither video will be available for viewing online after the concerts.
